The Board of the Numurkah District Health Service have approved the most significant health initiative to be introduced to the Numurkah district in the past ten years. With a focus on chronic disease prevention through a comprehensive health promotion program; early intervention for those at risk of chronic disease or newly diagnosed and helping chronic disease sufferers to better manage their disease, this project will seek to help and support the young, parents, families and the elderly to live healthier lives.

This is a fixed term position for a three year period. This is a significant opportunity for a highly motivated, tertiary qualified individual. We are looking for a suitable qualified and experienced person who is able to implement a new Intake and Chronic Disease Management model within Numurkah Community Health with a focus on:
- Improved Intake practices
- Improved management/coordination of clients with chronic and complex disease
- Improved integration of services between Community Health and General Ward
This role requires a person who is self motivated, proactive and able to function as a member of a multidisciplinary team. This position reports directly to the Manager Community Health.

Remuneration will be in accordance with the relevant EBA. Salary packaging is available. This position is part time. The successful applicant must have a current police check.
