Social / Support Worker
What you need to know about the Social / Support Worker:
- It is a free service.
- You can refer yourself or someone you know.
- Your doctor/hospital may also refer you.
- The Social / Support Worker is located at the Community Health Centre Monday to Friday between 9.00am – 3.00pm.
- You will need to make an appointment.
- The social worker does not tell you what to do they help you to find solutions to your situation.
- Your attendance is voluntary.
- Confidentiality is maintained at all times.
Social / Support Workers can:
Offer practical support around:
- Family changes
- Housing issues
- Financial worries
- Filling in forms
- Reading documents
- Understanding your health care
- Provide information
Counselling Support:
- Assist you to be in control of your situation.
- Help you find strategies to reduce stress.
- Work with you to change challenging behaviours.
- Support you through Loss and Grief (death, loss of job, health etc).
Group Work:
- Motivational Self Empowerment
- Stress Reduction
- Understanding and Living with a Mental Illness
Rights and Responsibilities
You have the Right to:
- Be treated with respect, dignity and the utmost care.
- Feel physically and emotionally safe at all times.
- Feel in control of you support plan.
- Receive quality care by the Social / Support Worker at all times.
- Make your own informed decisions.
- You have the right to ask to be referred to another health professional.
Your Responsibilities are:
- To be honest and upfront at all times with the Social / Support Worker.
- To attend your appointments at the scheduled time.
- Cancel appointments at least 12 hours in advance.
- Treat the Social / Support Worker with respect and dignity.
- Accept the outcomes of your own informed decisions.
- To ask questions about your care plan.
- Participate actively in your care plan.
