The District Nursing Service’s major objective is to ensure high quality professional nursing care for people with short or long term illnesses or disabilities who choose to remain in their own homes.
Referrals
Will be received from any person or organisation. Eligibility of each person will be individually assessed

Ancillary Services
Liaison with and referral to other HACC (Home and Community Care) and community services eg:
- Podiatry/Foot Care Clinic

- Meals on Wheels
- Home Help
- Community Nurse
- Hume Region Palliative Care
G.V. Rural Health Team:
- Occupational Therapist
- Physiotherapist
- Continence Advisor
- Hospital in the Home
- Post Acute Care
Equipment Loan
Assistance may be given for obtaining some items, however all equipment loan is for a maximum period of three months.
Fees
The Health Department of Victoria sets the fee for District Nursing visits.

Access to Services
Services are available to all members of our community, but some consumers may need to be assessed for specifically funded services. (CALD,ATSI, HACC,PAC etc)
